What Is Spravato®?
SPRAVATO® is a medication for people diagnosed with treatment-resistant depression and/or people diagnosed with major depressive disorder accompanied by suicidal ideation and/or suicidal behavior.
SPRAVATO® was first approved by the Food and Drug Administration (FDA) in 2019 for use in combination with another antidepressant. In 2025, the FDA expanded this decision, approving SPRAVATO® as a standalone treatment.
Patients self-administer SPRAVATO® exactly like a nasal spray used for allergies, with administration always and only occurring under direct medical supervision, with a required post-administration observation period of at least two hours.
SPRAVATO® is also effective and used off label for the treatment of typical major depressive disorder (MDD), post-traumatic stress disorder (PTSD), obsessive-compulsive disorder (OCD), anxiety disorders, and other mental health conditions.
What Spravato® Treats
SPRAVATO® helps reduce symptoms and negative psychological, emotional, and behavioral consequences of the following:
- Treatment resistant depression (TRD), defined as depression that does not improve after two attempts at treatment with standard antidepressants
- Major depressive disorder (MDD) with suicidal thoughts or behaviors
- Bipolar depression (BD-I & BD-II)
- Anxiety disorders (AD)
- Post-traumatic stress disorder (PTSD)/Trauma-related disorders
- Other mental health disorders
Treatment with SPRAVATO® is different than treatment with standard antidepressants for two reasons. First, it relieves symptoms quickly. Many patients report symptom reduction within an hour. Second, the effect is durable. Whereas standard antidepressants only reduce symptoms while a patient is taking the medication, the effects of SPRAVATO® can last for several weeks after treatment.
Evidence Base for Spravato®
Research shows that for people with treatment resistant depression, medications like SPRAVATO® – considered psychedelics – can improve the following:
- Hope for the future/optimism/confidence in treatment
- Self-esteem
- Overall mood and wellbeing
- Psychological resilience and adaptability
- Mindfulness
Follow-up studies show that SPRAVATO® – when combined with a typica antidepressant – leads to additional positive outcomes:
- Decreased symptoms of depression within 60 minutes
- Decreased symptoms of depression within 48 hours
- Ongoing reduction of symptoms of depression for longer than three weeks
- Increased duration before return of symptoms of depression
SPRAVATO® is safe in combination with typical antidepressants, atypical antidepressants, and psychotherapy. In most cases, patients use SPRAVATO® alongside other treatment, but after the upgraded approval in 2025, combination therapy is no longer required for SPRAVATO®
